Inclusive Development and Challenges of LGBTIQ+ Inclusion in Clinical Setting (110968)

Japan has been noted as lagging in understanding and addressing LGBT issues. Efforts are being made to enhance understanding of LGBT issues through school education and the enactment of related laws. However, healthcare field still faces challenges in establishing a ideal system for accommodating patients of LGBT. In this study, we aim to clarify nursing administrators’ knowledge and awareness of LGBT individuals and the level of consideration provided to LGBT patients during hospitalization. The survey was conducted via a web-based questionnaire. Responses were obtained from 107 nursing administrators from 100 randomly selected hospitals. Participants have not received education on gender diversity during school education, and 80 percent have not undergone any training after graduation. Many participants lacked sufficient knowledge of gender diversity, recognized the need for opportunity acquire knowledge. In terms of hospital policies, fewer than 10% of hospitals had standards addressing ethical considerations for gender diversity, such as using a patient's self-identified gender in medical records and documents, providing a transgender-specific rooms, or offering access to transgender-friendly restrooms. Many nursing administrators acknowledged that their LGBT-related knowledge was insufficient and expressed the need for further knowledge, although opportunities for learning were limited. Especially, they felt difficulty in determining visitation rights and consideration for being present at the time of death, and that they faced greater challenges in making decisions regarding gender identity, preferred names, selection of hospital rooms and restrooms. These results suggest the need for educational opportunities and establishment of organizational standards.
